What is HIFU?

High-Intensity Focused Ultrasound is a non-invasive way to treat uterine fibroids and adenomyosis. HIFU uses real-time ultrasound and high-intensity ultrasound waves to generate localized heat to target individual fibroids to destroy the cells specifically.

Using this treatment method, in conjunction with image guidance, the physician can heat and destroy the fibroid tissue without damaging nearby tissue or the tissues that the beam passes through on its way to the target.

01. What is ultrasound and focused ultrasound?

Ultrasound is sound waves that are transmitted at a frequency higher than 20,000 Hz per second and it is beyond the normal hearing range of humans.
Focused ultrasound is a noninvasive treatment therapy. Its working principle is to make use of the effective penetration of ultrasound waves through human tissues; the ultrasound beams generated by the ultrasound transducer outside the human body is precisely focused on the target lesion in the body, causing coagulative necrosis of the lesion by the conversion of mechanical effect into thermal and cavitation effect, thereby achieving non-invasive ablation of the lesion. It has been applied not only in the treatment of solid tumors, such as uterine fibroids, adenomyosis, liver tumors, pancreatic cancer, kidney tumors, bone tumors and breast tumors.

High intensity focused ultrasound ablation (HIFU ablation) is a new technology to ablate and kill the tumors in the body without cutting open or puncturing, so HIFU ablation is also called focused ultrasound ablative technology.

Its working principle is similar to using a convex lens to concentrate the sunlight which produces high temperature at the focal point that can lit the paper on fire. Ultrasound can also be focused and travels through the body safely. As a noninvasive therapy, HIFUfocuses the ultrasound energy outside the human body on the target lesion inside the body, producing high temperature (60 ℃ or higher) at the focal point instantly and causing irreversible coagulative necrosis of the target lesion, without damaging the tissues in the acoustic pathway and those surrounding the target lesion. The necrotic tissues will be gradually absorbed or become scars (fibrosis).

Haifu System refers to the Focused Ultrasound Tumor Therapeutic System Developed and manufactured by Chongqing Haifu Medical Technology Co., Ltd with independent intellectual property rights. There are several models of the Haifu System with differences in appearance and clinical indications.

HAIFU System is used to treat solid tumors (both malignant and benign), such as uterine fibroids, adenomyosis, liver tumors, pancreatic cancer, kidney tumors, bone tumors, and breast tumors.

Haifu system has been used in 27 countries and regions, including the UK, Germany, Spain, Egypt, Argentina, Singapore, Korea and Japan. Its safety and effectiveness in the treatment of benign and malignant tumors have been internationally recognized.

The John Radcliffe Hospital, one of Oxford University Hospitals, UK, introduced the Haifu System in 2002, becoming the first overseas user of the Haifu System outside China. The Minister of Health of Spain once appealed to list the Haifu System as the preferred option for the treatment of uterine fibroids and adenomyosis. And the University Hospital Mutua de Terrassa and the University Hospital Bonn use Haifu System as a major option for the treatment of such malignant tumors as pancreatic cancer. To date, there have been 21 HIFU centers in Korea using the Haifu System in the treatment of benign and malignant tumors; uterine fibroids, adenomyosis, liver cancer and pancreatic cancer have received Korean FDA approval.

As of December 2019, 151,975 patients have been treated with HIFU ablation performed using Haifu System, confirming the safety and effectiveness of HIFU ablation and HaifuSystem.

Haifu System has been used in clinical practice for over two decades with the number of patients treated reaching 151,975 as of December, 2019. Hundreds of academic papers have been published and have shown that the safety and effectiveness of HIFUtreatment (performed using Haifu System) for both benign and malignant tumors have been shown in hundreds of academic papers. For example, a large 20-center prospective cohort study ledbyProf. LANG Jinghe, Academician of Chinese Academy of Engineering and President of Chinese Obstetricians and Gynecologists Association (COGA), enrolling 2,411 patients with symptomatic fibroids across 20 Chinese hospitals, showed that HIFU ablation (n=1,353) hada similar effect to myomectomy (n=586) and hysterectomy (n=472), but a better safety profile with minimal injury, remarkable symptom relief, improvement of quality of life of patients, considerable shorter hospital stay and lower medical costs. A retrospective case series of

9,988 patients with uterine fibroids (7,438 patients) or adenomyosis (2,549 patients) showed that HIFU ablation for uterine fibroids and adenomyosis is highly effective and safe with light and temporary adverse reactions under conscious sedation, hence an alternative treatment for women with uterine fibroids and adenomyosis. A retrospective case series of 27,053 patients with uterine fibroids (n=17,402), adenomyosis (n=8434), caesarean scar pregnancy (n=876), or placenta accreta (n=341) showed that HIFU is safe in treating patients with benign uterine diseases with low rate of major adverse effects. In addition, HIFUablation for uterine fibroids has been included into the Guidance of National Institute for Health and Care Excellence (NICE Guidance), UK and other international clinical guidelines respectively by the American College of Obstetricians and Gynecologists (ACOG), theSociety of Obstetricians and Gynaecologists of Canada (SOGC), the Korean Society Of Obstetrics and Gynecology (KSOG), and the Chinese Obstetricians and Gynecologists Association (COGA).

The HIFU treatment performed using Haifu System is under the real-time guidance of ultrasound, which means the treatment effect can be seen in a real-time manner and thus the treatment scope and intensity can be adjusted according to the patient’s reaction to the treatment. In addition, as there is no need of general anesthesia or spinal anesthesia duringHIFU treatment of uterine fibroids and adenomyosis and a small amount of sedatives and analgesics will suffice, the patient can communicate with the medical professionals all along the treatment if any discomfort. And medical professionals are also by the side of the patient all the time observing vital signs and hence coping with any situations that may occur in a timely manner. And therefore, the HIFU treatment process is safe and reliable.
